สาเหตุของโรคความดันสูงสามารถแบ่งตามสาเหตุการเกิดได้เป็น 2 ประเภทใหญ่ ได้แก่
ชนิดที่ยังไม่ทราบสาเหตุที่แน่ชัด (Primary/Essential Hypertension)
โรคความดันสูงชนิดนี้มักเกิดขึ้นในวัยผู้ใหญ่เป็นส่วนใหญ่ โดยมีการพัฒนาของโรคอย่างค่อยเป็นค่อยไป และยังไม่สามารถระบุต้นเหตุที่ทำให้เกิดได้อย่างชัดเจน
ชนิดที่ทราบสาเหตุ (Secondary Hypertension)
โรคความดันสูงชนิดนี้มักเกิดขึ้นแบบเฉียบพลันมากกว่าชนิดแรก โดยเป็นผลมาจากโรคประจำตัวเดิม เช่น ภาวะหยุดหายใจขณะหลับ โรคไต ปัญหาต่อมไทรอยด์ เนื้องอกที่ต่อมหมวกไต โรคเบาหวาน โรคหลอดเลือดผิดปกติแต่กำเนิด
รวมไปถึงการตั้งครรภ์ การใช้สารเสพติดอย่างโคเคนหรือแอมเฟตามีน การติดแอลกอฮอล์และการติดสุราเรื้อรัง และการใช้ยาบางชนิด เช่น ยาคุมกำเนิด ยาลดน้ำมูก ยาลดไข้ และยาแก้ปวด
นอกจากนี้ ยังพบปัจจัยอื่นที่มีความเกี่ยวข้องกับการเกิดโรคความดันสูง ซึ่งส่งผลให้เกิดโรคได้มากขึ้น
อายุ อายุที่เพิ่มมากขึ้นจะยิ่งทำให้เกิดความเสี่ยงต่อโรคความดันสูงมากขึ้นตามไปด้วย โดยเฉพาะช่วงวัยกลางคนถึงวัยสูงอายุ ประมาณ 45–60 ปีขึ้นไป
เชื้อชาติ มักพบในคนเชื้อชาติฝั่งประเทศตะวันตกมากกว่าเอเชีย
เพศ มักพบในเพศชายวัยกลางคนอายุประมาณ 45 ปีขึ้นไป ขณะที่เพศหญิงจะพบมากในช่วงอายุ 60–65 ปีขึ้นไป
พันธุกรรม ผู้ที่มีประวัติคนในครอบครัวเป็นโรคความดันสูงมีโอกาสเป็นโรคได้สูงกว่าผู้ที่ไม่มีประวัติคนในครอบครัวเป็นโรคนี้
อาหารโซเดียมสูงหรือโพแทสเซียมต่ำ การรับประทานอาหารมีโซเดียมสูงจะยิ่งทำให้ร่างกายเกิดภาวะบวมน้ำ ซึ่งจะไปเพิ่มความดันโลหิตในร่างกายให้สูงขึ้น ส่วนอาหารที่มีโพแทสเซียมน้อยจะทำให้ร่างกายไม่สามารถจัดการกับปริมาณโซเดียมได้อย่างมีประสิทธิภาพ
เป็นโรคอ้วนหรือมีน้ำหนักเกินมาตรฐาน ผู้ที่มีปัญหาน้ำหนักเกินจะยิ่งมีความเสี่ยงมากขึ้น เนื่องจากระบบหมุนเวียนเลือดต้องขนส่งออกซิเจนและสารอาหารไปให้เนื้อเยื่อภายในร่างกายมากขึ้น หัวใจต้องใช้แรงดันในการส่งเลือดมากขึ้นเช่นกัน
สูบบุหรี่หรือยาสูบ สารพิษที่อยู่ในบุหรี่และยาสูบเหล่านี้จะเพิ่มความดันโลหิตในร่างกายให้สูงขึ้น ทำให้ผนังหลอดเลือดแดงเกิดการตีบตัน และหัวใจต้องใช้แรงดันในการส่งเลือดเพิ่มมากขึ้นกว่าปกติ นอกจากนี้ ผู้ที่ได้รับสารเหล่านี้จากควันบุหรี่ก็ได้รับผลเสียเช่นเดียวกับผู้ที่สูบ
ขาดการออกกำลังกายและเคลื่อนไหวร่างกายน้อย ทำให้อัตราการเต้นหัวใจเพิ่มมากขึ้น และยิ่งทำให้หัวใจต้องใช้แรงดันเพิ่มมากขึ้น
เป็นโรคเรื้อรังบางชนิด โรคประจำตัวบางชนิดอาจส่งผลต่อความดันโลหิตที่สูงมากขึ้น เช่น โรคไต โรคเบาหวาน หรือมีปัญหาด้านการนอน
